Avoid unplanned hospitalizations

Call9 decreases unplanned hospital utilization by over 50% for nursing home residents by bringing tailored acute care to their SNF bedside. Call9 takes a true population health approach by including all skilled nursing facility (SNF) residents—even those with the highest complexity—as part of our care model. We utilize Unplanned Hospitalization Rates (UHR) to establish benchmarks and measure success for a payor’s population residing in the SNF.

Reduce medical expense and share risk

Call9 establishes shared savings and shared risk arrangements that ensure payor/provider collaboration. By replacing both the ED visit and the subsequent hospitalization, Call9 saves the healthcare system $8 million annually per each 200-bed nursing home. Nearly 80% of all Call9 patient encounters, including treatment of high-risk patients requiring acute interventions, result in the resident staying in the SNF to receive their care. Residents who have an Emergency Severity Index Level of 4 and 5, which require less urgent and non-critical interventions, are treated in the SNF at a rate of over 95%.

The Medical Group is led by board-certified Emergency Medicine physicians who have additional training in geriatric, skilled nursing facility and prehospital care. The Group also includes board-certified Palliative Care physicians who assist residents in making medical decisions based on their preferences and work with primary teams and consultants alike to ensure medical plans align with residents’ values. Call9 physicians track a rigorous set of 17 Value Measures—which cover quality, cost and efficiency and include opportunities for patient-reported feedback—to ensure the highest quality care and establish controls that safeguard against care stinting and patient cherry-picking. Routine monitoring and analysis of defined quality indicators ensure early identification of undesired trends.
