Patient-Driven Payment Model (PDPM)

Recent changes in the skilled nursing industry have placed a greater burden on facilities to treat, manage and maintain higher levels of acuity. CMS’s new Patient-Driven Payment Model (PDPM), which will be implemented in October 2019, incentivizes resident complexity and will base payment on patient characteristics, such as diagnosis and comorbidities. Value-Based Purchasing (VBP) Program

Approximately one in five patients arriving at a SNF is transferred back to the hospital within 30 days. Call9’s on-site Clinical Care Specialists and virtual physicians provide an extra layer of support for facility staff, adding diversity of skill and giving the facility another resource in caring for residents. Facility nurses are extremely skilled in chronic care; however, Call9’s presence in the facility gives them additional confidence and comfort in treating patients in-house when acute emergencies occur.
