Founded in Silicon Valley by an Emergency Medicine physician and now headquartered in Brooklyn, Call9’s tech-enabled, human-powered solution identifies patients who are at the highest risk for hospitalization and cares for them in the comfort of their nursing home beds. Our goal is to reduce the cost of nursing home patients to the healthcare system while improving patient outcomes and increasing access to care.

Managing patients’ needs across a continuum of care

Creating a model of care that reduces unnecessary senior hospitalizations has enabled us to manage both the financial and clinical risk associated with this high-risk population. Call9’s interdisciplinary care team engages a proactive, data-driven approach to provide palliative services, optimize patient care plans and treat acute emergencies as they arise.

Humanizing telemedicine to administer care at the bedside

Our hybrid care model embeds emergency-trained first responders—generally an EMT or paramedic—on-site in nursing homes, 24 hours a day, 7 days a week, 365 days a year. As patients get sicker, the paramedic goes to the bedside and utilizes proprietary technology to immediately connect to Call9’s remote Emergency Medicine physician and advanced practice provider care team via our virtual care platform.

Using data analysis and machine learning techniques to identify patients at risk

We bring an enhanced suite of emergency diagnostics to the bedside for every patient encounter, delivering clinical tools to nursing homes that were traditionally only available in a hospital setting. Through integrations with all of the major nursing home EMRs, Call9’s powerful technology platform captures real-time data that enables improved medical decision making and provides a higher quality of care and a better experience for the patient, family and facility staff.

Closing the gaps in care delivery by focusing on value over volume

Our value-based approach reduces unnecessary Emergency Department visits, hospitalizations and subsequent readmissions in nearly 80% of patient encounters.
